Ginkgo biloba, more commonly known as ginkgo, has been used medicinally for over 1000 years. The ginkgo tree is the world's oldest living tree species.

Ginkgo was originally utilized by traditional Chinese physicians for a variety of problems including asthma and digestive disorders. More recently, ginkgo leaf extract has been utilized for its antioxidant properties, for a number of vascular problems, and for the treatment of memory loss, dementia, and macular degeneration. Ginkgo represents one of the most studied and commonly used herbal remedies in the world. The National Health Interview Survey found that ginkgo was the third most popular natural product in the United States in 2002, used by 4 percent of American adults. Over 400 animal and clinical trials have been performed looking at a variety of medicinal properties and clinical uses. Ginkgo Biloba is most commonly used to treat the elderly for disorientation, depression, memory loss, headaches, tinnitus, and vertigo because of its positive effects as a circulatory aid. It is thought to help increase blood flow to the brain, which makes it a possible treatment for cerebral insufficiency.

Those suffering from other circulation problems, such as leg cramps, have found relief from using Ginkgo Biloba. Although there are no conclusive studies, Ginkgo’s antioxidant effects may protect the cardiovascular the central nervous system from the effects of aging such as mental deterioration. Ginkgo is thought to also restrain high blood pressure.

Mechanism of action — It is thought that there are two main groups of active constituents responsible for ginkgo biloba's medicinal effects: terpene lactones and ginkgo flavone glycosides, which are present in varying concentrations in the leaf of the ginkgo tree. Approximately 40 different flavonoids have been isolated, including ginkgetin, bilobetin, and sciadopitysin. Terpenes isolated include a number of ginkgolides and diterpenes, most importantly the ginkgolides A, B, and C along with bilobalide.

The bioactive properties of ginkgo biloba extracts are varied. Ginkgo biloba extract appears to alter neurotransmission and provide neuroprotection via a number of mechanisms demonstrated in animal models, including decreased glucose utilization in brain areas mediating somatosensory processing and vigilance. Furthermore, ginkgo reduces the number of peripheral adrenal benzodiazepine receptors, altering corticosteroid secretion. Ginkgo biloba extract also has been shown to protect against age-related changes in the hippocampus. Other studies indicate that ginkgo biloba extract reversibly inhibits monoamine oxidase A and B.

Other studies examining the mechanism of action include:

  • Animal studies have found that the terpene lactones inhibit the binding of platelet-activating factor (PAF) to its membrane receptor. Ginkgolides also have a protective effect against PAF-induced bronchoconstriction and airway hyperactivity. Antagonism of PAF has been studied in animal models looking at improved myocardial contractility and coronary flow.
  • Ginkgo biloba extracts demonstrate antioxidant properties, resulting in reduced superoxide release in polymorphonuclear cells. Flavone glycosides including quercetin, kaempferol, and isorhamnetin are also noted to have antioxidant properties and inhibit platelet aggregation (blood clotting). These actions are thought to protect brain tissue against hypoxic damage and improve cerebral metabolism. In particular, the constituents of ginkgo may act as scavengers for free radicals, which are likely mediators of the lipid peroxidation and cell damage observed in Alzheimer disease.
  • Ginkgo may increase cholinergic transmission in the brain via inhibition of acetylcholinesterase; this may have further implications for Alzheimer disease therapy.
  • The vascular effects of ginkgo are thought to be due to vasodilation secondary to stimulation of the endothelium derived relaxing factor and prostacyclin release. Other studies suggest that ginkgo biloba may inhibit nitric oxide, thereby leading to vascular relaxation.
  • Ginkgo extracts lower the serum fibrinogen concentration
